The client base for Together in Transition covers a wide range of business types and individuals. We work closely with high quality independent, assisted living and nursing care residential facilities, as well as doctors’ offices in gerontology and geriatrics and local hospitals. We also maintain strong relationships in the community with local churches, synagogues and senior centers.
 
Individuals clients come to us in several ways: directly, when they are independently making the decision to ‘downsize’ and move into a simpler, smaller space, as well as through family members, who are supporting a loved one requiring a safer, higher-level care residence than their current situation allows.
