The client base for Together in Transition covers a wide range of business types and individuals. We work closely with high quality independent, assisted living, board and care and nursing care residential facilities, as well as doctors’ offices in gerontology and geriatrics and local hospitals. Providing complementary services to elder law, estate planning and accounting professionals is a core aspect of our business model. Enhancing the breadth and quality of  benefits with the industry leaders in the long-term care insurance sector is growing. We also maintain strong relationships in the community with local churches, synagogues and senior centers.

Individuals clients come to us in several ways: directly, when they are independently making the decision to ‘downsize’ and move into a simpler, smaller space, as well as through family members, who are supporting a loved one requiring a safer, higher-level care residence than their current situation allows. Financial planners and accountants with long-time relationships with their clients call on Together in Transition to provide expertise in the transition planning process as the client make life decisions about their aging preferences: residential options, support systems to prolong independence, access to community resources and more. And residential communities refer prospective clients to our services for the overwhelming process of downsizing and moving from a residence of 40+ years into a smaller, communal-based setting. We also provide educational presentations to medical staff and discharge planners in understanding transition care options that can improve the quality of patient care as well as reduce return patient visits to the hospital.
